HIEMT Pro: Building Muscle and Burning Fat in 30 Minutes
Most body-shaping technologies focus on fat. HIEMT does both - it builds muscle and reduces fat at the same time, in one 30-minute session. At Soma Laser & Skincare Clinic in Pretoria, the HIEMT Vertical 4-Handle Pro is the latest addition to our body-sculpting menu, and it has rapidly become one of the most asked-about treatments on the schedule.
The science
Muscle accounts for roughly 35% of body weight, but most slimming devices ignore it entirely. HIEMT (High-Intensity Electromagnetic Therapy) uses focused electromagnetic energy to trigger up to 20 000 supramaximal muscle contractions per session - far more intense than anything voluntary effort can produce.
Voluntary muscle activation, even at maximum effort, recruits roughly 30 to 35 percent of the available muscle fibres in any single contraction. HIEMT bypasses the brain-muscle signal pathway and directly stimulates the motor neurons, recruiting close to 100 percent of fibre population in each contraction. The fibres respond by adapting (growing stronger and larger) the same way they respond to heavy resistance training - just compressed into an intense 30-minute window.
Published research consistently demonstrates, on average:
- 16% increase in muscle mass in the treated area
- 19% reduction in subcutaneous fat in the same area
- Improved diastasis recti closure in postpartum studies
- Improved bladder support and reduced stress incontinence in pelvic floor protocols
Where it works best
HIEMT is ideal for the abdomen, glutes, biceps, triceps, thighs and calves. Two of its most popular use cases at our clinic are:
- Post-pregnancy diastasis recti - pulling separated rectus abdominis muscles back together and rebuilding core strength
- Glute lift - non-surgical Brazilian-style toning for clients who do not want fillers or surgery
Other areas that respond well include the upper arms (a common request from women noticing arm laxity in their forties and fifties), the thighs (especially the inner thighs where voluntary training is hardest), and the calves for clients with limited calf development.
What a session feels like
You lie down, the applicators are strapped on, and intensity ramps up gradually over the first few minutes. Then 30 minutes of intense involuntary contractions - like the world's most efficient ab class while you scroll your phone. The contractions are visible: you can see your own abs flexing without trying. Mild muscle soreness for a day or two afterwards is normal, and a sign it is working.
The intensity is adjustable. First-time clients usually start at around 50 to 60 percent and ramp up over subsequent sessions as the muscles adapt. By session three or four, most clients tolerate 90 to 100 percent intensity comfortably.
How many sessions?
A course of 4 to 6 sessions over 2 to 3 weeks delivers the strongest initial results. Twice-weekly is the standard cadence during the active phase. Maintenance once a month thereafter holds the gains. Many clients run a course twice a year - winter prep and summer maintenance.
What you should see
- Sessions 1-2 - intense muscle awareness, visible flexing during treatment, mild post-session soreness
- Sessions 3-4 - subtle visible change in muscle tone, particularly noticeable on the abdomen
- Sessions 5-6 - measurable contour change, clothes fitting differently, especially around the waist
- Weeks 4-8 after the course - peak results as the muscle adaptation continues to develop and the fat reduction becomes visible
Who it suits
HIEMT is best for clients within a healthy weight range who want definition and core strength rather than dramatic fat loss. It is not a substitute for fat freezing or weight management - it is a complement. Many clients combine HIEMT with Cooltech: HIEMT for muscle definition, Cooltech for stubborn fat pockets.
It also suits:
- Postpartum mothers rebuilding core strength
- Clients with abdominal separation after pregnancy
- Athletes looking for accelerated muscle development in specific areas
- Clients in their 40s and 50s seeing muscle tone reduction with age
- Anyone wanting a non-surgical glute lift
Diastasis recti specifically
Diastasis recti - the separation of the rectus abdominis muscles along the linea alba - affects up to two-thirds of women postpartum. Traditional core exercises can sometimes worsen the separation if done incorrectly. HIEMT activates the rectus abdominis evenly along its length, drawing the muscles back together and rebuilding the core function. Postpartum clients typically wait until at least three months after delivery (six months after a caesarean) before starting HIEMT.
Contraindications
HIEMT cannot be used if you have:
- Pregnancy
- Pacemakers, defibrillators or other implanted electronic devices
- Metal implants in the treated area (joint replacements outside the field are usually fine)
- Recent surgery in the treated area
- Active cancer
- Hernia in the treated area
- IUDs (some types) - we screen carefully at consultation
- Pulmonary insufficiency or recent cardiac event
Our therapists screen carefully at the consultation. The contraindication list reads as long, but the vast majority of clients are eligible.
HIEMT for stress incontinence and pelvic floor
The HIEMT Vertical 4-Handle Pro can also be configured for pelvic floor stimulation, addressing stress incontinence and post-pregnancy pelvic floor weakness. Sessions in this configuration are the same length but with the client seated rather than lying. Studies show meaningful improvement in stress incontinence symptoms after a six-session course, with continuing benefit at the six-month follow-up.
Combining with other treatments
HIEMT pairs well with:
- Cooltech fat freezing (alternating weeks)
- Radiofrequency skin tightening (different sessions, complementary actions)
- Manual lymphatic drainage massage (helps clearance after both fat freezing and HIEMT)
- Personal training (HIEMT activates the muscle, voluntary training reinforces the pattern)
