Laser Tattoo Removal: What Actually Happens to the Ink
Tattoo regret is more common than the industry admits. Surveys consistently show that one in four people with a visible tattoo wishes they had not got it. The good news is that modern Q-switched laser technology removes most tattoos completely - but it is not fast, and the colour of the ink dictates the timeline.
Tattoo removal is one of the more honest treatments in the aesthetic space. The mechanism is well-understood, the timelines are predictable, and the limits are clearly defined. A reputable clinic will tell you exactly what to expect, including the inks that may not clear completely.
The mechanism
Q-switched lasers fire ultra-short pulses of light at specific wavelengths. The energy is absorbed by the tattoo ink and shatters it into microscopic particles. Your body's immune system - specifically macrophages - then clears those fragments through the lymphatic system over the weeks following each session.
This is why patience matters. The fading happens between treatments, not during them. The session itself only sets up the next month or two of clearance. Pushing sessions closer than the recommended interval does not speed up clearance and can cause unnecessary skin trauma.
Wavelengths and colour targeting
Different ink colours absorb different wavelengths of light:
- 1064 nm (Nd:YAG) - black, dark blue, dark brown
- 532 nm (frequency-doubled Nd:YAG) - red, orange, brown
- 755 nm (alexandrite) - green, blue
- 694 nm (ruby) - green, blue
- Picosecond technology - newer-generation lasers that work across colours with shorter pulse durations and less surrounding tissue stress
A clinic with a single wavelength will struggle on multi-coloured tattoos. Black ink clears most reliably. Greens, yellows and pastels are the most difficult.
Realistic session counts
- Black ink: 6 to 10 sessions, 6 to 8 weeks apart
- Red and dark blue: 8 to 12 sessions
- Green, yellow, and pastel colours: often need specialist wavelengths and may not clear 100%
- Cosmetic tattoos (eyebrows, lip liner): handled differently due to oxidation risk - some pigments oxidise to a darker colour on first laser pass and require careful test patching
- White ink: notoriously difficult; titanium dioxide in white ink can darken on laser exposure rather than fade
What affects the timeline
Smokers clear ink slower because circulation is reduced. Sun-exposed skin needs to fade before treatment. Older tattoos generally clear faster than fresh ones because the body has already partially broken down some of the ink. Professional ink is denser than amateur ink - and clears more predictably. Tattoos placed close to lymph nodes (e.g. inner upper arm, neck) clear faster than tattoos on the lower legs and feet, where lymphatic flow is slower.
Health and lifestyle factors that improve clearance:
- Good hydration
- Regular cardiovascular exercise (improves lymphatic flow)
- Adequate sleep (immune system performs clearance during sleep)
- Not smoking
- Healthy diet without excess inflammatory foods
What it feels like
Most clients describe each pulse as a hot rubber band snap. A medium tattoo (palm-sized) takes 10 to 20 minutes per session. Larger pieces are sometimes split across two appointments to manage the discomfort. Topical anaesthetic cream applied 45 minutes before the session takes the edge off significantly. Mild swelling and pinpoint frosting on the skin afterwards are normal - both settle within a few hours.
Test patching
For cosmetic tattoos, white ink, or tattoos with unknown ink composition, a small test patch is performed two to four weeks before the first full session. The test patch identifies any unusual response (paradoxical darkening, allergic reaction, slow clearance) before committing to a full course. This is standard care for any cautious clinic.
Aftercare that actually matters
Keep the area clean and dry for 24 hours, no swimming for two weeks, and apply SPF 50 every day until the area has fully faded. Sun exposure is the single biggest cause of poor results.
Detailed aftercare:
- Day 1 - keep the area clean and dry, apply the prescribed antibiotic ointment
- Days 2 to 7 - gentle washing with non-fragrant soap, dressing changes daily, no scratching of any blisters or scabs that form
- Weeks 1 to 4 - daily SPF 50 on the area, no swimming or sauna, no sun exposure
- Weeks 4 to 8 - the body continues to clear the ink; the tattoo gradually fades
- At week 6 to 8 - assessment for the next session if more clearance is needed
Possible side effects
- Temporary redness and swelling
- Pinpoint bleeding or blistering, particularly on darker pigments
- Temporary lightening (hypopigmentation) of the surrounding skin, usually resolving over months
- Less commonly, persistent hypopigmentation or hyperpigmentation
- Allergic response to dispersed ink (rare but possible with certain red and yellow pigments)
- Scarring if aftercare is neglected or the laser is operated incorrectly
Cosmetic tattoos and microblading removal
Cosmetic eyebrow tattoos and microbladed brows can be removed, but the chemistry of the iron oxide pigments commonly used can cause paradoxical darkening on the first laser pass. We always test patch and use a conservative protocol. Saline tattoo removal (a lift-out technique) is sometimes a better option for cosmetic tattoos and is offered as an alternative.

Who should not have it
- Pregnancy and breastfeeding
- Active infection in or near the tattoo area
